Understanding the Four Main Parts of Medicare

Medicare can feel overwhelming for newcomers, with four parts offering different coverage types. Understanding what each part covers helps you choose the right plan for your health needs.

HealthPlans of NC

Medicare consists of four main parts, each covering different healthcare needs. Here's a breakdown of what each part offers and how they work together.

Medicare Part A: Hospital Insurance

  • Covers inpatient hospital stays, including semi-private rooms, meals, and general medical services.

  • Includes skilled nursing facility care (limited days), hospice care, and blood transfusions (beyond the first 3 pints).

  • Premium-free for most people if you or your spouse worked and paid Medicare taxes for at least 10 years (40 quarters).

  • If you haven't met the work requirement, you can buy coverage (partial premiums possible for 30–39 quarters).

Medicare Part B: Medical Insurance

  • Covers outpatient services, including doctor visits, preventive care (e.g., screenings, vaccines), lab tests, surgeries, ambulance services, and durable medical equipment.

  • Some hospital-based services (e.g., doctor fees) fall under Part B.

  • Requires a monthly premium ($185 standard in 2025; higher for high-income individuals).

  • Recommended if Medicare is your primary coverage.

Original Medicare = Parts A + B. Enroll through Social Security (online at SSA.gov, by phone, or in person).

Medicare Part C: Medicare Advantage

  • Private plans approved by Medicare that bundle Parts A, B, and often D (prescription drugs).

  • Usually include extra benefits like dental, vision, hearing, fitness programs, and transportation.

  • Most plans use provider networks (HMO or PPO) with lower costs for in-network care.

  • Premiums vary by plan, location, and provider; many have $0 premium (beyond Part B).

  • Eligibility requires enrollment in Parts A and B and living in the plan’s service area.

Medicare Part D: Prescription Drug Coverage

  • Helps pay for outpatient prescription drugs.

  • Offered through private plans (standalone or included in Medicare Advantage).

  • Costs vary by plan, including monthly premiums, deductibles, and copays.

  • Check the plan’s formulary (drug list) and pharmacy network.

  • Enroll during your Initial Enrollment Period or Annual Enrollment Period to avoid late penalties.

Choosing the Right Coverage for You

  • Basic coverage

    — Enroll in Parts A and B; add Part D for prescriptions.

  • More comprehensive

    — Consider a Medicare Advantage plan (often includes dental/vision and Part D).

  • Minimal out-of-pocket

    — Add a Medigap (Supplement) plan to Original Medicare.

  • Always ensure your medications are covered on the formulary to avoid high costs.
